Referrals ( April 2024 Dr Sissons is currently not accepting New Patient Referral's)

 

Before making an appointment your child will need a referral letter from your doctor.

 

Please email your GP referral letter to deakin@specialistservices.com.au

 

GP referrals are generally valid for 12 months only.  After 12 months, you will need your GP to make a new referral.  You will also need a new GP referral letter if you are attending for a new problem.

 

Please be aware that referrals provided by a specialist are only valid for 3 months.

 

If you do not have a valid referral you will not be able to claim from Medicare for your consultation.

 

Next Steps:

 

After we have received your referral, we will contact you and let you know what you need to do.

 

In general, we will ask you to complete a Paediatric Questionnaire which provides us with background information and enables us to focus on the issues that concern you most.

 

Once we have received the completed questionnaire we will contact you with some preliminary information about your appointment, fees etc.  At times, there may be a considerable wait for appointment. Our receptionists will bring your appointment forward if there are cancellations

 

What to bring to your first appointment:

 

  1. Medicare card

  2. Your child's Health and Development Record ("Blue Book")

  3. Any previous assessments or reports for your child (e.g. hearing assessment, letters from other specialists, speech/occupational therapy/psychology/ developmental/ cognitive assessments and school reports

 

Please arrive with your child 10 minutes prior to the time booked for your first appointment.
